change your password immediately, and choose a good one this time (few people do)

That in blue is just telling you who made the request. Which is you. The reason is if someone else was trying to get your password, the request would be sent to your email and you would know who was trying to hack your account (or at least that somebody was).

The address and hostname is your computer (I assume) and has nothing to do with your password. Also in that email should be your account name and recovered password. If it's not the password you created, use it to log in and then change your password to something safe.

try your account on another computer, it sounds like a cookie problem or somthing.
